Sulphamethoxazole + Trimethoprim
Compound trimethoprim has a bactericidal effect on a variety of Gram-positive and Gram-negative bacteria in vitro, including Streptococcus, Staphylococcus, Pneumococcus, Neisseria, Pasteurella catarrhalis, Escherichia coli, Klebsiella , Proteus, Haemophilus, Salmonella, Shigella, Brucella. ., Nocardia and Bordetella. It has particularly high activity against Haemophilus influenzae, Escherichia coli and Proteus, which makes compound trimethoprim particularly suitable for the treatment of chronic bronchitis and urinary tract infections. Compound trimethoprim plays its bactericidal effect by sequentially blocking two bacterial enzyme systems in the biosynthesis of folinic acid in microorganisms. The resulting synergy explains the high bactericidal activity. Indications are :
Cotrimoxazole has broad bactericidal action against gram-positive and gram-negative bacteria, as well as certain protozoa. The bactericidal effect of Co-trimoxazole, which contains Trimethoprim and Sulphamethoxazole in a 1:5 ratio, is mediated by the successive blockage of two bacterial enzyme systems involved in the production of folinic acid in the bacterium.
The following doses of Levamisole are given as a single administration, preferably after a light meal.
In cases of severe hookworm infection it is suggested that a second standard dose be given one or seven days after the first, whichever timing is feasible.
Crystalluria, allergic responses, haemolysis, thrombocytopenia, neutropenia, agranulocytosis, and other adverse effects have been described seldom with the combination of Sulphamethoxazole and Trimethoprim. Malaise, headache, nausea, and vomiting are some of the less significant side effects. These are usually transitory and do not need therapy discontinuation.
Because sulphonamides travel through the placenta and are secreted in breast milk, they can induce kernicterus during pregnancy and breastfeeding.
Because the medication interferes with the conversion of Folic acid to Folinic acid, prolonged full-dose therapy with the sulfamethoxazole-trimethoprim combination increases the risk of macrocytic anemia. Folinic acid can be given to reverse this if it happens. When providing this combination to diabetic patients on a sulphonylurea medicine, it's important to keep in mind that the sulphonylurea's effect may be potentiated.
