Acute symptomatic relief of angina pectoris; prophylactic management in situations likely to provoke angina attacks; long-term prophylactic management of angina pectoris due to coronary artery disease.
Nitroglycerin promotes vascular smooth muscle relaxation, resulting in a vasodilator action on both peripheral arteries and veins. Venous dilation promotes peripheral blood pooling and reduces venous return to the heart, lowering left ventricular end-diastolic pressure (preload), while arterial relaxation lowers systemic vascular resistance and arterial pressure (afterload).
Dosage should always to be adjusted according to the requirement and response obtained by the individual patient and the severity of the anginal pain. For adults, one Nitroglycerin 2.6 mg sustained released tablet or capsule in morning and evening. This should be taken empty stomach.
Nitroglycerin SR may potentiate the effects of peripheral vasodilators and may cause severe hypotension when used along with Sildenafil.
In patients who have a known allergy to nitroglycerin, other organic nitrates, or nitrites, or to the medicine's excipients, nitroglycerin is contraindicated. Acute myocardial infarction, severe anemia, head trauma, cerebral haemorrhage, or closed angle glaucoma are other contraindications.
Headache, dizziness, weakness, mild gastrointestinal symptoms may occur. They disappear on dosage reduction and usually do not necessitate the discontinuation of the treatment.
There is no evidence relating to the safety of nitrates in pregnancy and lactation. Therefore, Nitroglycerin SR may be used in pregnancy and lactation only when considered essential by the physician.
Regular use of Nitroglycerin SR (three times daily) should be avoided because of the developing of tolerance. Two times daily administration and withdrawal for a short period may re-establish the original sensitivity.
